A charge from SMK*SURVEYMONKEY on your statement

SMK*SURVEYMONKEY.COM is SurveyMonkey, the online survey company. Its paid plans renew by themselves, most of them once a year on the same date, so the usual answer is a yearly plan someone signed up for and forgot.2 SMK*WUFOO.COM is Wufoo, an online form builder that belongs to SurveyMonkey and is billed the same way.5

The short answer

Sign in at surveymonkey.com, open My Account, then the Billing Details tab, and turn auto-renew off. That stops the next charge, not this one.1 SurveyMonkey’s terms call its payments "non-refundable" and say it will not refund once the account has been used to collect responses, unless the law requires it or SurveyMonkey decides to. Ask anyway, and ask quickly: Delaware’s card records show SurveyMonkey refunds, half of them within six days of the charge.46

What it looks like on a real statement

SurveyMonkey’s help pages do not say what its charges look like on a statement. The State of Delaware publishes every purchase its employees make on state cards, with the merchant name as the bank received it. When we counted on 1 October 2026 it had 1,223 lines for SurveyMonkey and Wufoo, from 28 June 2017 to 24 August 2026, bought by state departments, school districts and colleges.6

SurveyMonkey and Wufoo lines in Delaware’s card data, 2017–2026
What the statement said Lines When
SMK*SURVEYMONKEY.COM 861 28 June 2017 to 17 August 2026, still in use
SURVEYMONK* T 47616739 107 1 August 2021 to 24 August 2026, alongside the line above. Always 8 digits, and a different number on every charge
SURVEYMONKEYUS 15 27 August 2024 to 15 June 2026, including one office charged $99 on the 9th of each month from July 2025 to January 2026
SURVEYMONKEY INC. 15 March 2021 to March 2026, larger amounts ($237 to $12,892)
SMK*SURVEYMONKEY 10 February to May 2018 only
SMK*WUFOO.COM CHARGE 215 2 July 2017 to 3 August 2026: Wufoo. 198 of the 215 were between $14.95 and $39

The number after SURVEYMONK* T is not your account number: the same office’s plan shows a new one each year. If your statement shows it, compare it, the date and the amount with the charges listed in your SurveyMonkey account.6 The Delaware data shows posted charges on one set of cards; your bank may shorten the line or add a town and phone number to it.

Why it came on the same date as last year, and for more

SurveyMonkey’s auto-renew page: "Most plans auto-renew on an annual basis (every 12 months) on the same day each year." If you pay by card, "the payment method listed in the Billing Details tab is automatically charged at your next billing date". The same page says that customers who pay by invoice are emailed the invoice 45 days before; it describes no such email for card payments.2

The Delaware data shows both things. One unit of the Department of Safety and Homeland Security was charged $468 on 14 June in 2023, 2024, 2025 and 2026. An office of the Department of State was charged $384 on 13 September 2022 and then $468 on 13 September 2023, 2024 and 2025, about a fifth more for the same renewal date.6 SurveyMonkey’s terms allow price changes at the end of a billing period, with "advance notice".4 A higher amount than last year is not by itself a sign of fraud.

Why a new card was charged

SurveyMonkey says that if your bank has sent you a replacement card, "you may see the charge posted to your new card": the bank can accept a charge made to the old card details and put it on the new one. Changing the card in your account can also place a temporary hold on it, which "should be automatically removed" within about 7 business days.3 Cancelling the old card is therefore not a reliable way to stop a renewal. Turning auto-renew off is.

How to find it and stop it

  1. Find the account Search your email, including spam, for “SurveyMonkey” and for the exact amount. A survey set up for a school, a club, a church or a former job may have used your card. If you bought the plan in SurveyMonkey’s iPhone or iPad app, Apple bills it, not SurveyMonkey, and you cancel it in your Apple subscriptions.1
  2. Turn off auto-renew Sign in, select your profile circle at top right, then My Account, the Billing Details tab, Manage Subscription and Downgrade to Basic, and confirm. You keep the paid plan until the date shown and are not charged again. Only the person who upgraded, or a team’s Primary Admin, can do this. If there is an unpaid invoice, or the account was set up through SurveyMonkey’s sales team, the option is not there and you have to contact SurveyMonkey.1 For Wufoo: Account (or Account Settings), View Billing Information, then Disable Auto-Renew.5
  3. Ask for the refund before you delete anything Contact SurveyMonkey from its help centre and say that the charge was a renewal you did not want. Do not delete the account first: deleting it means you "agree to forfeit the remaining time" and "won’t be refunded for this unused time".1
  4. If nobody you know bought it, go to your bank If there is no SurveyMonkey or Wufoo account you or anyone near you can find, treat it as fraud: ask your bank to cancel the card and dispute the charge, and ask what its deadline is. It depends on the bank, the card network and the reason, and missing it can end your options.

What SurveyMonkey says about refunds

SurveyMonkey’s terms of use, effective 27 December 2025, say: "You agree to pay us for our Services and these payments are non-refundable." Cancelling in the middle of a billing period brings a refund only if SurveyMonkey has broken the terms and not fixed it within 30 days, if the law requires one, or if SurveyMonkey "in our sole discretion" decides a refund is appropriate. And: "we will not grant a refund where you have used our Services, collected responses, and/or downloaded your responses" unless one of the first two applies.4 The help centre says the same in fewer words: "previous charges won’t be refunded unless legally required". Consumers in the European Union have separate refund rights.1 Wufoo’s help page uses the same sentence.5

Refunds do happen. The Delaware data has 20 SurveyMonkey refunds, each matching an earlier charge of the same amount to the same office. Ten came back within six days of the charge; the slowest took 79 days.6 The data does not show why any of them was refunded.

Is this a scam?

The name is real: every one of these lines is SurveyMonkey’s own, and SurveyMonkey in the United States is SurveyMonkey Inc., 910 Park Pl, Suite 300, San Mateo, California.46 That does not make every such charge wanted. A stolen card can pay for a survey plan as easily as anything else. If no account anyone you know can sign in to has the charge, it is not a renewal you forgot, and it belongs with your bank.

Why this page is on a site about held money

This site records payment companies that hold back money that belongs to someone else. SurveyMonkey is not in the incident record; this page is here so that people searching for a name on a statement find the answer. A yearly renewal from a website or domain company looks like DNH*GODADDY; software sold for another company often shows up as PADDLE.NET or FS * or FSPRG; a stock photo plan that renewed by itself is STK*SHUTTERSTOCK, and a newspaper is GAN*NEWSPAPER SUB or USAT, and a deposit for jail phone calls is GTL*INMATE PHONE SVC. A ChatGPT plan shows up as OPENAI *CHATGPT SUBSCR, and Creative Cloud or Acrobat as ADOBE *ADOBE or ADOBE INC. An email newsletter plan shows up as MAILCHIMP *MISC or MAILCHIMP. Other lines are in the table of statement names.
